If your health insurance does not provide coverage, you may elect to pay for these services directly. Dr. Provencal will complete any paperwork required by your insurance company to submit for reimbursement. Some insurance companies will reimburse a portion of the assessment.

A referral and/or prior authorization may be required. Co-payments and deductibles may apply. We encourage patients to verify coverage with their insurance company. Insurance does not cover evaluations solely to assess for Learning Disabilities or for Educational Testing. There will be a separate charge for the portion of the assessment that examines academic abilities. Participation in school meetings, school observations, hearings and mediations are not covered by insurance.
